This deposit payment is to enable  your facilitation in a study on the Pharma Hermetic Hair Recovery Programme which has been initiated by the company to help to provide more information that provides context to the before and after photographs.  The programme is described on the following page Pharma Hermetic Hair Recovery Programme – Crewe Hair and Skin Clinic 01270 747 393  and is essentially a cosmetic topical solution administered with a 0.5mm dermaroller device. No injections are involved, and the treatment is not expected to be painful although some mild discomfort may be experienced

You will be able to receive treatment at a heavily discounted price which includes:

• 1 shampoo worth £44.99
• 1 hair lotion worth £149.99
• 1 TrichoLab images (one at the beginning and end of the 6 treatments) worth £150

The usual cost of a treatment package

You can use this link to pay a deposit if you are a female over the age of 18, have had a non-scarring form of hair loss diagnosed at this clinic, and are able to attend the clinic on 6 consecutive occasions (or as close as possible) 10 days apart (including Sundays).

The forms of hair loss eligible for this study include:

• Female Pattern Hair Loss
• Telogen Effluvium
• Early Alopecia Areata
• Early Traction Alopecia

Unfortunately scarring forms of hair loss including Lichen Planopilaris, Frontal Fibrosing Alopecia and CCCA are not eligible for this study, but can still access treatment separately

Before making payment, please could you ensure that you have reviewed the consent form to participate in this study, as well as the consent form for each treatment?

Once the payment has been received you will be sent a link which you  can use 6 times to book in a date in the diary. If there is a clash you will be informed as soon as possible.  Please can you book in your slots to start within 2 weeks of payment?

Please note that if you withdraw your photographic consent you will be invoiced for the cost of the treatments you have already received.

If you complete the treatment programme and feedback as agreed, the amount that you paid will be refunded to you.
